The Humber and North Yorkshire Cancer Alliance (HNYCA) are prioritising making health inequalities data useful for Primary Care. Over the past few months we have explored what data is currently available to PCNs and practices, what they are using, and how they are using it. We are trying to build a picture of how this looks across Primary Care and what it could look like to better support proactive management of health inequalities.
